A UK dental note example, from consent to signature
Follow one recall appointment through Squte: the consent question, the recording, the note written while the dentist sees the next patient, a change by voice, and the signed note copied into practice software.
Made-up patient. This is an illustration of how Squte works, not a recording of a real appointment.
What a Squte note looks like
A Squte note follows the 17 sections UK dentists use, from chief complaint to plan and recall, with UK tooth notation (UR6). It fills in only what the appointment covered and leaves the rest blank. The dentist checks it, changes it by voice and signs it before it goes anywhere.
Step 1 of 8
Ask first
Before recording, the dentist reads Squte’s consent line to the patient.
The patient’s answer is saved with the visit: the wording used, how it was given and the time. Squte will not transcribe a visit without it.
If the patient says no, nothing is recorded. You type the note, or build it with chairside chips, as you would without Squte.
“We use an AI note-taker that records our conversation to write up your notes. It’s kept secure and processed in the UK and EU, and you can say no. Are you happy with that?”
Step 2 of 8
Record the appointment
The dentist treats as normal. Squte transcribes as they talk.
A short voice sample, recorded once, lets Squte tell the dentist’s voice from the patient’s, so each line is labelled correctly.
Note the diagnosis: the dentist says it out loud. Squte records a diagnosis only when you state one.

- DentistTry to clean between your teeth, and cut down the fizzy drinks between meals. We’ll book the filling and see you in six months.
Step 3 of 8
Stop, and call the next patient
The dentist taps Stop. Squte has transcribed as it went, so only the last few seconds are left to process.
There is nothing to wait for. The next patient is already in the chair while the note is written.

Step 4 of 8
The note is ready
Squte says when the note is ready to check. The dentist can open it between patients or at the end of the session.
Until it is signed, it stays a draft.

Step 5 of 8
Read the 17-section note
The note follows the 17 sections UK dentists use, in order, with UK tooth notation: “upper right six” became UR6. Social history and treatment delivered today are left blank, because the appointment didn’t cover them.
Then check it. Here the radiograph grade is missing, because the dentist didn’t say one. That is what your check is for.

Step 6 of 8
Change it by voice
The dentist taps Refine, says the change and taps Apply.
Only the section they asked about changes. The rest of the note stays as it was.
Advice
Clean between the teeth daily. Reduce sugary drinks between meals. Interdental brushes, size 3, shown for the lower molars.
The other 16 sections are unchanged.
Step 7 of 8
Sign it
Nothing goes into the record until the dentist signs it.
The draft, each change and the signature are logged with who did it and when.
- Draft written by Squte09:42
- Advice changed by voice09:58
- Signed by the dentist09:59
Nothing goes into the record until you sign. Every change is logged with who made it and when.
Step 8 of 8
Copy it into practice software
One tap copies the whole signed note. The dentist pastes it into Dentally, Software of Excellence, R4 or any other system.
There is no direct integration yet: it is one copy and one paste.
Chief complaint: Sensitivity to cold, upper right, for two weeks. History of presenting complaint: Pain with cold drinks only. No pain on biting. No night pain. Medical history: Checked: no changes to health or medicines. No known allergies. Dental history: Brushes twice daily. No interdental cleaning. …

What this example shows
- “Upper right six” became UR6: teeth are written in UK notation, or FDI if the dentist prefers.
- Two sections are blank because the appointment didn’t cover them. Squte doesn’t fill gaps with stock phrases.
- The diagnosis is in the note because the dentist said it. Squte never suggests one.
- The radiograph grade is missing because it wasn’t said. Every note needs your check before you sign.
- Nothing reached the record until the dentist signed it.
See your own note on your own appointments.
14 days or 100 visits, whichever comes first. Card required, and nothing is charged during the trial. Then from £25 a month, no VAT to add. Cancel before the trial ends and you pay nothing.
